2. Please review and fax back the Supplier Data form, along
with your Professional Services Agreement, to
1-866-512-7251.
Once we have received your signed
Professional Services Agreement and completed
Supplier Data Form, you will be instantly
enrolled in our durable medical equipment (DME)
service.
Once you have submitted your enrollment
forms, one of our representatives will promptly
contact you to answer any questions you may have
and discuss all the benefits our DME service has
to offer you and your patients. You will be
provided with the necessary form which you will
need to complete so that we can begin to
pre-authorize your patients and for you to begin
earning your Professional Service and Component
Fee. Your DME service representative will also
explain the minimal paperwork involved, multiple
product line selection and what your
Professional Service and Component Fee will be
per patient fitting, per product.
